Care Coordination & Resources
Not sure where to start? That's exactly what we're here for. Our intake team will listen, understand your needs, and connect you to the right services.
Whether you're a parent seeking help for your child, a professional making a referral, or a community partner looking to connect a family with support — our intake team is the front door to everything we do. We screen, assess, and provide warm handoffs to the services and resources that are the best fit for each family.
We listen to your concerns and assess the family's needs through thoughtful conversation.
We identify the right level of care — from prevention and early intervention to clinical treatment.
We provide warm handoffs to internal programs or community resources that match the family's needs.
We check in to make sure families are connected and getting the support they need.
A Warm Welcome
Our intake team is more than a phone line — they're skilled professionals who take the time to listen, understand, and guide each family to the services that are the best fit. Every connection begins with care.
When a family reaches out — whether through a referral from a pediatrician, school, or community partner, or by contacting us directly — our intake team begins with a thoughtful conversation. We ask about the child's needs, the family's circumstances, and what kind of support would be most helpful.
From there, we provide a warm handoff to the right program or service. That might be one of our own programs — like our Child and Family Center, school-based therapy, infant mental health services, Child First, or SNAP® — or it might be a community resource that better fits the family's needs.
Care coordination runs through everything we do. Beyond our intake team, our Pediatric Integration Program includes a dedicated care coordinator embedded within partner pediatric practices, and our Child First teams each include a care coordinator who connects families with housing, employment, health care, and other essential supports.
Within our A Way of Being with Children approach, helping a family access basic needs is an act of care that communicates: "You are not alone, and you deserve support." This is what it means to help families feel Safe and Seen.
Whether you submit a referral through our website, connect through a partner portal, or reach out by phone or email — a member of our intake team will follow up personally. There's no wrong way to get started, and no concern too small.
We'll take the time to understand the situation and what kind of support would be most helpful. From there, we'll guide you to the right program or resource.
Kelly leads our intake team, ensuring every family receives a compassionate, responsive experience from the very first contact. She coordinates referrals, screenings, and connections across all of our programs.

"Connecting families to resources — housing, food, childcare, mentoring, and behavioral health services — helps create the safe, stable conditions that children need to thrive."
— Guided by the HOPE Framework & A Way of Being with ChildrenA Public Health Approach to Intake
Our intake process is guided by the same public health model that runs through every program we offer. We don't just match families to services — we assess the right level of care based on each family's unique strengths, needs, and circumstances.
Universal education, awareness, and resources that build protective factors and positive childhood experiences for all children and families — including those who are doing well and want to stay on a healthy path.
Targeted support for families showing early signs of concern — behavioral challenges, developmental questions, family stress, or emerging needs. We intervene early, before challenges become crises.
Evidence-based clinical services for children and families with significant mental health needs — delivered in the office, school or childcare center, pediatric offices, the client's home, in the community, and via telehealth.
Screening for Strengths, Not Just Risk
Our intake process is grounded in the HOPE framework — which means we don't just screen for adversity. We also screen for the positive childhood experiences and protective factors that help children thrive.
Safe, stable, nurturing connections with caring adults
Places that are physically and emotionally safe
Social, civic, and community connection
Social and emotional competence and development
Research shows that positive childhood experiences can buffer against the effects of adversity — even when ACEs are present. That's why our intake team screens not only for adverse experiences but also for the positive experiences and protective factors that build resilience. This strengths-based approach helps us see the whole picture and connect families to services that build on what's already working.
